- Purchase $50 worth of ANY P&G products between 12/14/08 and 1/31/09.
- Circle products purchased and price paid on your original receipts.
- Print the P&G form and fill out.
- Place in a stamped envelope, enclose the form and original dated sales receipt with item descriptions and product purchase prices circled.
